KLES HOSPITAL: ONE MORE SURGICAL MILESTONE BY THE DEPARTMENT OF SURGICAL GASTROENTEROLOGY

17 years Female presented to KLES Dr Prabhakar Kore Hospital, Belagavi with recurrent episodes of vomiting of blood for the past 10 years and failure of growth since childhood. She was diagnosed with a very complicated disease called EHPVO (extrahepatic portal venous obstruction). She visited multiple hospitals but did not find relief.

Later she also complained of vague left-sided pain abdomen caused by which she stopped going to college. On evaluation, she had a massive splenomegaly extending beyond the umbilicus as shown in the picture. On evaluation her blood count was very less, Hb was 4 mg/dL, and platelets count was 40,000 and TLC of 2500 cells/cumm.

On CT scan of the abdomen, she had portal cavernoma with a patent splenic vein of 1.2cm and left renal vein of 9mm with multiple large venous collaterals measuring max diameter of 2.2cm and a massive spleen of 35cm x 20cm.

She underwent a complicated major surgery of 8 hours – splenectomy with splenorenal venous shunt following which her low blood counts improved in the immediate post op period with resolution of symptoms.

This major & rare surgery was successfully handled by Dr Sudarshan B Chaugule, Consultant Surgical Gastroenterologist & his team at the Department of Surgical Gastroenterology, KLES Dr Prabhakar Kore Hospital, Belagavi.

Now patient has gained weight, is healthy and has started going to college.
